Breast cancer is notoriously easy to metastasize at the early stage. Despite the fact that breast is an external organ easy to detect cancer at the early stage to achieve lifelong cure and the availability of multiple therapeutic modalities, breast cancer remains a top killer. The objective of this article is to attribute inability of cancer authorities to handle unhealed wound and cancer stem cells (CSCs) as the primary reasons for the failure of breast cancer therapy and to offer cell differentiation agent (CDA) formulations as the only option to heal the unhealed wound and to eradicate CSCs, which are critically linked to unhealed wound, to win the war on breast cancer. Cancer evolving due to wound unhealing was a valid concept of cancer introduced by R. Virchow in 1858. Western medicine is totally unprepared to handle diseases evolving due to wound unhealing. Western medicine likes to solve diseases based on the elimination of symptoms. Perpetual proliferation of cancer cells (CCs) is the most outstanding symptom of cancer. Thus, killing of CCs became the favored approach of cancer therapies, and the disappearance of tumor became a requirement of cancer drugs and a standard diagnosis on the success of cancer therapy. Both were wrong, because killing of CCs to create wound clearly violated the advice of Virchow. Since killing of CCs was incorrect for cancer therapy, disappearance of tumor was also incorrect for the evaluation of cancer therapy. We pursued cancer therapy unknowingly to follow the advice of Virchow to discover abnormal methylation enzymes (MEs), chemo-surveillance and the mechanism of wound healing. Wound healing requires the proliferation and the terminal differentiation (TD) of progenitor stem cells (PSCs). PSCs are the most primitive stem cells to initiate the development of organs and tissues during the embryonic stage of fetal development. So, wound healing is an extension of embryonic program of organ and tissue development. PSCs express telomerase, MEs of telomerase expressing cells are abnormal due to the great affinity of MEs and telomerase to form complex, which changes kinetic properties of MEs and the regulation of cell growth greatly in favor of the promotion of cell growth. Wound healing comes naturally, because the nature creates chemo-surveillance and immuno-surveillance to ensure perfection of wound healing. Human body produces cell differentiation agents (CDAs) to function as chemo-surveillance, which is selectively destroyed in cancer patients for cancer to evolve and to progress. The restoration of chemo-surveillance is very critical to the success of breast cancer therapy, which can be achieved by the administration of CDA formulations. CDA-2 was a preparation of wound healing metabolites we purified from urine for cancer therapy, which has been approved by the Chinese FDA for the therapy of breast cancer as an adjuvant agent to supplement cytotoxic cancer therapy and as a mono-therapeutic agent for the therapy of myelodysplastic syndromes, which are diseases attributable entirely to the propagation of CSCs. Since CSCs play a very prominent role on the pathology of breast cancer displaying to cause metastasis at the early stage and the emergence of triple negative breast cancer cells to resist therapies currently available, CDA formulations are naturally the standard care of breast cancer.
